Introduction

MedCor4U is polymetric non-invasive health monitoring mobile application available on both iOS and Android devices. Its purpose is to connect you with your doctor and health professional in a seamless manner. You can measure your own readings in the comfort of your home and your readings will be sent to your healthcare provider.

The Med Company B.V (further TMC) is the manufacturer of MedCor4U.

TMC has selected CE certified medical devices to be integrated with the MedCor4U app so that you can take your own readings efficiently and accurately to report the results to the internal system of the hospital accessed by the doctor and health professionals associated with your specific case.

TMC’s aim is to create a secure telemonitoring system to connect you – as a patient – and your readings directly to your healthcare professional and hospital’s Electronic Health Records (EHR) or any other health systems authorised by your healthcare provider.

TMC has followed a strict criteria to select only CE certified, high quality home monitoring devices such as contactless thermometers, blood pressure, saturation and weight scale monitors. The result is MedCor4U mobile based application with secure connectivity to various devices and at the same time user friendly.

TMC’s mission is helping you to receive guideline based therapies at home using CE certified monitoring devices, connecting your vitals to your healthcare provider in a secure way.

Intended Use

Following are the intended use of MedCor4U:

MedCor4U is not a replacement for therapy or treatment and does not provide any therapeutic treatments, instead it is complementary to the healthcare services and allows the users to measure their readings comfortably from home
Usage is limited to people who can read and literate in one of the languages supported by the App. Currently the App supports both English and Dutch languages
Mild colour blindness and mild visual impairments can use the app without additional assistance
Certain devices have limitations. Refer to the instructions for use – supported device chapter for more info

IMPORTANT: Contact your healthcare provider if you are not feeling well or if your symptoms get worse.

MedCor4U system comprises of:

Mobile App: Available on both iOS and Android, it allows the measuring equipment to connect wirelessly to the app. With one click you can send readings, view simple charts or read information about your condition
MedCor4U cloud: Provides a secure platform for patients, doctors and health professionals to access personalised care recordings

By using the MedCor4U telemonitoring system you will be connected to medical staff via the Electronic Health Record (EHR) or any other health systems authorised by your healthcare provider using a secure connection.

1. Use of data within TMC

TMC handles two types of data:

Your personal information: When your healthcare provider registers you with MedCor4U
Your health data: When you sync your readings from the medical devices. This data is then shared with your doctor and healthcare professionals

Why data exchange

TMC created a secure telemonitoring system to connect patients and their vitals directly to their healthcare professional and hospital’s Electronic Health Records (EHR) or any other health systems authorised by their healthcare provider. By using the MedCor4U telemonitoring system your readings will be shared with your medical staff.

We take privacy very seriously, the exchange of this data conforms to the requirements of the General Data Protection Regulation (GDPR).

Principles

The TMC team considers the proper protection of personal data to be of the utmost importance. Our principles are:

Personal information always remains yours. You can always ask your healthcare provider to see, change or delete your data
Before you can start using any of our products, we always ask you to read our privacy statement and agree on the terms
We do not sell your personal information
We only use your data to make our application work properly

2. How does TMC protect your data

The TMC team is very aware of the privacy sensitivity of all data processed within the organisation. TMC handles your information securely and in accordance with the policies and the laws and regulations in the Netherlands.

TMC has taken various technical and organisational security measures. This protects your data against loss, misuse, alteration, distribution, destruction, theft and against access or use by people who do not have permission to do so.

In our organisation we pay attention to the importance of data protection and information security:

Technical measures have been implemented to secure your data<

Following are some of the measures taken:

Data is encrypted in the database. People who want to do harm can’t do anything with this encrypted data as they don’t have the key used for decrypting the data
Data is always transferred over secure HTTPS (SSL) connection
MedCor4U users Sign In with their user name and their own created password. Passwords are stored encrypted and no one apart from the user can obtain this password (even TMC technical team)
Firewall is used for any online service available on the internet
Disaster recovery and backup best practices are followed by having nightly and incremental backups throughout the day to ensure there is no loss of data
Personal data of the EU users of MedCor4U is stored in EU based data centre
A regular check is carried out on the access rights to ensure the maximum privacy
Access to all systems are logged and controlled
A designated limited number of persons are authorised and in charge of carrying out the Processing of Personal Data
TMC has concluded processing agreements with its suppliers and clients. The parties associated with TMC comply with the necessary certifications and/or work at least according to TMC’s standards

Quality and quality marks

MedCor4U is EU-MDR CE certified. Furthermore, MedCor4U is deployed on the AWS infrastructure services that are ISO 27001 certified.

3. Who can view your data

TMC staff do not have automatic access to personal data. Only Help Desk staff and very limited staff who have been given an explicit authorisation can access your personal data when necessary.

In order to ensure your data is not kept longer than necessary, all applicable laws and regulations are respected.

5. Detailed information on the use of your data

5.A For what purpose do we collect the data

Your data will only be used to allow MedCor4U to provide you with the intended service. This means, for example, that a measurement captured using MedCor4U is sent to a doctor or healthcare professional.

Your healthcare provider chooses to use MedCor4U as part of your treatment. This means that TMC has an agreement with the hospital where you are being treated. In order to carry out this agreement it is necessary that your personal information is shared with TMC.

5.B What data do we collect

By the use of the MedCor4U your (medical) readings will be exchanged for the purpose of diagnosis – it is a continuous monitoring process that allows a healthcare professional to remotely interpret the data necessary for medical follow-up and, if necessary, make decisions regarding the state of your health.

We only collect the minimal data to allow MedCor4U to operate properly, If you do not wish to provide this necessary data then it will not be possible to use MedCor4U.

Following is the type of data collected:

Your first and last name
Your Email address
Your Technical unique (patient) identification number when linked to the EHR (Electronic Health Record)
Your captured vital signs through the MedCor4U app and CE certified medical devices, such as contactless thermometers, blood pressure, saturation and body composition monitors

5.C Where does the data come from

You will need an account created for you before you will be able to use MedCor4U. Your healthcare provider will initiate your account and that information will be shared with TMC. These may originate directly from the hospital’s EHR or may have been filled in by the healthcare provider.

To ensure optimal use of the MedCor4U and for the benefit of your treatment, medical details and readings (the vital signs) are kept in the MedCor4U system.

5.D To whom do we provide the data

Your data may be viewed by the doctors and health professionals involved in your treatment and which have access to this data.

Your personal data will never be passed on to third parties, such as healthcare insurance companies.

6. Your rights

You have the right to view, modify or delete your data
You have the right to withdraw your consent to the use of your data.
You have the right to object to the use of your data
You also have the right to transfer data. This means that you can ask for your personal data to be sent to you or another healthcare organisation

For more information about your rights or the procedure for submitting a request, you can contact the Data Protection Officer of the hospital, GP or home care organisation where you are being treated.

If you wish to have your data deleted or if you withdraw your consent to its use, future data will not be processed.

U kunt uw eigen metingen meten in het comfort van uw huis en uw metingen worden verzonden naar uw zorgverlener.

TMC heeft CE-gecertificeerde medische apparaten geselecteerd om te worden geïntegreerd met de MedCor4U app, zodat u uw eigen metingen efficiënt en nauwkeurig kunt uitvoeren om de resultaten te rapporteren aan het interne systeem van het ziekenhuis dat toegankelijk is voor de arts en gezondheidswerkers die bij uw specifieke geval betrokken zijn.

Het doel van TMC is om een beveiligd telemonitoring systeem te creëren om u – als patiënt – en uw metingen direct te verbinden met uw zorgprofessional en het elektronische patiëntendossier (EPD) of andere zorgsystemen die door uw zorgverlener zijn goedgekeurd.

TMC heeft strenge criteria gehanteerd om alleen CE-gecertificeerde, hoogwaardige thuis bewakingsapparatuur te selecteren, zoals contactloze thermometers, bloeddruk-, saturatie- en weegschaal monitoren. Het resultaat is MedCor4U mobiele applicatie met veilige connectiviteit naar verschillende apparaten en tegelijkertijd gebruiksvriendelijk.

De missie van TMC is u te helpen om thuis op richtlijnen gebaseerde therapieën te ontvangen met behulp van CE-gecertificeerde bewakingsapparatuur, waarbij uw vitale functies op een veilige manier met uw zorgverlener worden verbonden.

Beoogd gebruik

Hieronder volgt het beoogde gebruik van MedCor4U:

MedCor4U is geen vervanging voor therapie of behandeling en biedt geen therapeutische behandelingen, in plaats daarvan is het een aanvulling op de gezondheidszorg en stelt het de gebruikers in staat om hun metingen comfortabel vanuit huis te meten
Het gebruik is beperkt tot mensen die kunnen lezen en lezen in een van de talen die door de App worden ondersteund. Momenteel ondersteunt de app zowel de Engelse als de Nederlandse taal
Lichte kleurenblindheid en lichte visuele beperkingen kunnen de app zonder extra hulp gebruiken
Bepaalde apparaten hebben beperkingen. Raadpleeg de gebruiksaanwijzing – hoofdstuk over ondersteunde apparaten voor meer informatie.

BELANGRIJK: Neem contact op met uw zorgverlener als u zich niet goed voelt of als uw symptomen verergeren.

Het MedCor4U systeem bestaat uit:

MedCor4U cloud: Biedt een veilig platform voor patiënten, artsen en gezondheidswerkers om toegang te krijgen tot gepersonaliseerde zorgregistraties

Als u het MedCor4U telemonitoring systeem gebruikt, wordt u via het elektronisch patiëntendossier (EPD) of andere door uw zorgverlener toegestane zorgsystemen met het medisch personeel verbonden via een beveiligd verbindingssysteem.

1. Gebruik van gegevens binnen TMC

TMC verwerkt twee soorten gegevens:

Uw persoonlijke gegevens: Wanneer uw zorgverlener u aanmeldt bij MedCor4U
Uw gezondheidsgegevens: Wanneer u uw metingen van de medische apparaten synchroniseert. Deze gegevens worden vervolgens gedeeld met uw arts en zorgverleners.

Waarom gegevensuitwisseling

TMC heeft een beveiligd telemonitoring systeem gecreëerd om patiënten en hun vitale functies rechtstreeks te verbinden met het elektronisch patiëntendossier (EPD) van hun zorgverlener en het ziekenhuis of andere zorgsystemen die door hun zorgverlener zijn goedgekeurd. Door gebruik te maken van het MedCor4U telemonitoring systeem worden uw metingen gedeeld met uw medisch personeel.

Wij nemen privacy zeer serieus, de uitwisseling van deze gegevens voldoet aan de eisen van de General Data Protection Regulation (GDPR).

Persoonsgegevens blijven altijd van u. U kunt uw zorgverlener altijd vragen uw gegevens in te zien, te wijzigen of te verwijderen.
Voordat u een van onze producten kunt gaan gebruiken, vragen wij u altijd onze privacyverklaring te lezen en akkoord te gaan met de voorwaarden.

2. Hoe beschermt TMC uw gegevens

Het TMC-team is zich zeer bewust van de privacygevoeligheid van alle gegevens die binnen de organisatie worden verwerkt. TMC behandelt uw gegevens veilig en in overeenstemming met het beleid en de wet- en regelgeving in Nederland.

TMC heeft diverse technische en organisatorische beveiligingsmaatregelen genomen. Deze beschermen uw gegevens tegen verlies, misbruik, wijziging, verspreiding, vernietiging, diefstal en tegen toegang of gebruik door personen die daar geen toestemming voor hebben.

In onze organisatie besteden wij aandacht aan het belang van gegevensbescherming en informatiebeveiliging:

TMC zorgt voor duidelijke afspraken over het melden van informatiebeveiliging gebeurtenissen.

Er zijn technische maatregelen getroffen om uw gegevens te beveiligen

Hieronder volgen enkele van de genomen maatregelen:

Gegevens zijn gecodeerd in de database. Mensen die kwaad willen, kunnen niets doen met deze versleutelde gegevens omdat ze niet beschikken over de sleutel die wordt gebruikt om de gegevens te ontsleutelen.
Gegevens worden altijd overgedragen via een beveiligde HTTPS (SSL) verbinding.
MedCor4U gebruikers melden zich aan met hun gebruikersnaam en hun eigen aangemaakte wachtwoord. Wachtwoorden worden versleuteld opgeslagen en niemand anders dan de gebruiker kan dit wachtwoord bemachtigen (zelfs niet het technische team van TMC).
Firewall wordt gebruikt voor alle online diensten die op het internet beschikbaar zijn.
Disaster recovery en back-up best practices worden gevolgd door nachtelijke en incrementele back-ups gedurende de dag om ervoor te zorgen dat er geen gegevens verloren gaan.
Persoonlijke gegevens van de EU-gebruikers van MedCor4U worden opgeslagen in een in de EU gevestigd datacentrum.
De toegangsrechten worden regelmatig gecontroleerd om maximale privacy te garanderen
Toegang tot alle systemen wordt geregistreerd en gecontroleerd
Een beperkt aantal personen is gemachtigd en belast met de verwerking van persoonsgegevens.
TMC heeft met haar leveranciers en opdrachtgevers verwerkersovereenkomsten gesloten. De aan TMC verbonden partijen voldoen aan de noodzakelijke certificeringen en/of werken ten minste volgens de normen van TMC.

Kwaliteit en keurmerken

MedCor4U is EU-MDR CE gecertificeerd. Verder wordt MedCor4U ingezet op de AWS infrastructuur diensten die ISO 27001 gecertificeerd zijn.

3. Wie kan uw gegevens inzien

Medewerkers van TMC hebben niet automatisch toegang tot persoonsgegevens. Alleen Helpdeskmedewerkers en een zeer beperkt aantal medewerkers die een uitdrukkelijke machtiging hebben gekregen, kunnen indien nodig toegang krijgen tot uw persoonsgegevens.

4. Hoe lang bewaren wij uw gegevens

Om ervoor te zorgen dat uw gegevens niet langer dan nodig worden bewaard, worden alle toepasselijke wetten en voorschriften nageleefd.

5. Gedetailleerde informatie over het gebruik van uw gegevens

5.A Voor welk doel verzamelen wij de gegevens

Uw gegevens worden alleen gebruikt om MedCor4U in staat te stellen u de beoogde dienst te leveren. Dit betekent bijvoorbeeld dat een met MedCor4U vastgelegde meting naar een arts of zorgverlener wordt gestuurd.

Uw zorgverlener kiest ervoor om MedCor4U te gebruiken als onderdeel van uw behandeling. Dit betekent dat TMC een overeenkomst heeft met het ziekenhuis waar u wordt behandeld. Voor de uitvoering van deze overeenkomst is het noodzakelijk dat uw persoonsgegevens met TMC worden gedeeld.

5.B Welke gegevens verzamelen wij

Door het gebruik van de MedCor4U worden uw (medische) metingen uitgewisseld ten behoeve van de diagnose – het is een continu monitoringsproces dat een zorgprofessional in staat stelt om op afstand de gegevens te interpreteren die nodig zijn voor de medische follow-up en, indien nodig, beslissingen te nemen over de toestand van uw gezondheid.

Wij verzamelen alleen de minimale gegevens om MedCor4U goed te laten functioneren, indien u deze noodzakelijke gegevens niet wenst te verstrekken zal het niet mogelijk zijn MedCor4U te gebruiken.

Hieronder volgt het type gegevens dat wordt verzameld:

Uw technische unieke (patiënt) identificatienummer indien gekoppeld aan het EPD (elektronisch patiëntendossier)
Uw vastgelegde vitale functies via de MedCor4U app en CE-gecertificeerde medische apparatuur, zoals contactloze thermometers, bloeddruk-, saturatie- en lichaamssamenstelling monitor.

5.C Waar komen de gegevens vandaan

Er moet een account voor u worden aangemaakt voordat u MedCor4U kunt gebruiken. Uw zorgverlener initieert uw account en die informatie wordt gedeeld met TMC. Deze kunnen rechtstreeks uit het EPD van het ziekenhuis komen of door de zorgverlener zijn ingevuld.

Voor een optimaal gebruik van de MedCor4U en ten behoeve van uw behandeling worden medische gegevens en metingen (de vitale functies) in het MedCor4U-systeem bewaard.

5.D Aan wie verstrekken wij de gegevens

Uw gegevens kunnen worden bekeken door de artsen en gezondheidswerkers die betrokken zijn bij uw behandeling en die toegang hebben tot deze gegevens.

Uw persoonsgegevens worden nooit doorgegeven aan derden, zoals zorgverzekeraars.

6. Uw rechten

U heeft het recht om uw gegevens in te zien, te wijzigen of te verwijderen.
U heeft het recht om uw toestemming voor het gebruik van uw gegevens in te trekken.
U heeft het recht om bezwaar te maken tegen het gebruik van uw gegevens.
U heeft ook het recht om gegevens over te dragen. Dit betekent dat u kunt vragen om uw persoonsgegevens naar u of een andere zorgorganisatie te sturen

Voor meer informatie over uw rechten of de procedure voor het indienen van een verzoek kunt u contact opnemen met de functionaris voor gegevensbescherming van het ziekenhuis, de huisarts of de thuiszorgorganisatie waar u wordt behandeld.

Als u uw gegevens wilt laten verwijderen of als u uw toestemming voor het gebruik ervan intrekt, worden toekomstige gegevens niet meer verwerkt.
